Request for a reference letter from me

I will gladly consider writing a reference letter for you if you have worked on a research project with me, or you have taken an upper-year course from me and you had a grade of at least A-, or you were placed among the top 20% of the class. Otherwise, I will not be able to write a letter that is of much benefit to you. If I do agree to write a letter for you, please provide me with the following information.
  1. Your university transcripts.
  2. Your resume or curriculum vitae.
  3. Your statement of interests for graduate studies if applying for graduate school.
  4. All the required forms that I need to fill out. (Please have the parts you need to complete filled out.)
  5. A deadline for completing the letter. (You should give me at least 2 weeks notice.)
  6. Instructions on where to send the reference letter.
  7. Any other relevant information about your abilities.
